Like the title says how could this happen without anyone knowing or helping Cypher back out or the Matrix?

Well, taking in count Mouse's comment about arraging a meeting with the red dress lady, I think that is possible to autoschedule a plug in and plug out, I mean, too much technology? That would be something possible to do in that scenario, but I guess it was not necessary going into detail about that. Thats my speculation of course, but I always had that same question.

But, how did they know that neo was going to touch the glass during the waking up scene?

Wouldnt that have been through the training program like in Neo's first encounter? That is not actually the Matrix, just a simulation/ training program Mouse created and Mouse would still have to help Neo in and out
